Driver SMS consent form

Carriers: print this, let the driver read it and choose, and keep it in their driver qualification file. Only if they tick Yes and sign do you record consent on their driver record in TruDOT — a separate step, after the record exists, that changes nothing else about the file. Consent may never be a condition of employment, and the driver can withdraw it at any time by replying STOP.

Your choice — tick one

Either answer is fine. Declining has no effect on your employment, your pay, your dispatch, or your driver qualification file, and you can change your mind later.

I agree to receive text messages from TruDOT about my DOT compliance documents and deadlines at the mobile number I provided. Message frequency varies. Message and data rates may apply. Reply STOP to cancel or HELP for help. Consent is not a condition of employment or of any purchase. See trudot.app/legal/messaging and trudot.app/legal/privacy.

  • What you’ll get: reminders when one of your documents — medical card, CDL, annual review — is coming due, and a confirmation when a document you text in has been filed. Never marketing.
  • How often: frequency varies with what is expiring; most drivers get fewer than six messages a month.
  • To stop: reply STOP to any message, or tell your office. To get help, reply HELP, email support@trudot.app, or call (425) 675-5732.
  • Your number: used only to send these messages and to match documents you text in to your file. It is never sold or shared with anyone for marketing.
Umapachi LLC · 11335 NE 122nd Way, Suite 105, Kirkland, WA 98034, United States · support@trudot.app · trudot.app/legal/messaging · Messages come from the TruDOT number your fleet was issued. Keep the signed original in the driver qualification file.
